To obtain the certification for the repatriation of bodies or ashes at the Consulate General of Italy in Melbourne, you will have to book an appointment through our online portal Prenot@mi<\/a> (“Consular Services”).<\/p>\n

Requested documents (see below) must be sent to us via email (melbourne.legale@esteri.it)<\/a> at least 2 weeks in advance and presented in original at your appointment.<\/p>\n

\nDOCUMENTS REQUIRED FOR THE REPATRIATION OF BODY REMAINS<\/strong><\/summary>\n
    \n
  • Copy of the last passport held by the deceased or Italian Identity Card<\/li>\n
  • Medical certificate in which it is stated that the deceased was not afflicted by any contagious disease<\/li>\n
  • Coroner\u2019s report with translation in Italian\u00a0(in the case of a violent death)<\/li>\n
  • Original death certificate with Apostille and translation into Italian<\/li>\n
  • Letter from the Funeral Director indicating:\n

    • Name, Surname of the deceased (maiden surname for females)<\/li>\n
    • Date and place of birth<\/li>\n
    • Last residential address of the deceased<\/li>\n
    • Details of the applicant that requests the repatriation<\/li>\n
    • Flight details<\/li>\n
    • Place of burial\/ final destination of the body remains<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n

      The original death certificate will be kept by the Consulate General whereas all other documents will be returned to the applicant.<\/p>\n<\/details>\n
